Your Nurse is With You Every Step of the Way

A special relationship for you and your baby

When you enroll in Nurse-Family Partnership (NFP) in your first pregnancy or as someone who is experiencing a high-risk pregnancy, you will meet someone who is devoted to you and your baby’s good health. An experienced nurse will visit your home or somewhere else if you prefer, at a time convenient for you.

During visits, your nurse will check in to see if there are any health issues related to your pregnancy or with your baby, and answer your questions with trusted information. Together, you will work on goals that are important to you and your family, and choose the topics that you want to discuss. Your nurse will also ensure that you are connected to benefits and services to best support you and your family.

Some visits will take place over the phone or by video call, and they usually last about an hour. Parents stay in the program until the baby turns two years old, and many participants remain with a single nurse throughout the program. The connection and useful information that many moms receive helps them become more confident parents.

As your baby approaches two years of age, your nurse will help you assess your baby’s development and plan for what comes next for your family, including Family Village programming that can provide ongoing support.

My nurse made sure I always had essentials such as diapers and milk. My therapist helped me with postpartum depression and PTSD. I didn’t believe I was able to be a good single mother, but my nurse, Grace, gave me hope and allowed me to fall in love with my pregnancy, understand the process, and work on the areas I’m not so good at.

My baby is already born. Am I still eligible for a personal care nurse with Nurse-Family Partnership?

In order to be eligible for Nurse-Family Partnership, an individual has to enroll during their pregnancy.  Please reach out to us to see if you can enroll in Healthy Start instead!

I recently found out that I’m pregnant with my first baby. Would Nurse-Family Partnership be the right program for me?

Nurse-Family Partnership in Delaware County is available to anyone who is pregnant with their first baby and is below a certain income level. NFP can also help someone who is in a second or subsequent pregnancy if they are below a certain income level and are also high-risk. Reach out to our Family Village staff to find out if you are eligible.

I am pregnant, and I am in recovery from a substance use disorder. I want to make sure that I stay healthy so that my baby can be healthy. Can Nurse-Family Partnership help me?

Yes. Nurse-Family Partnership is an excellent resource for pregnant women who have a substance use disorder or are in recovery. Nurse home visitors can provide you with medically accurate information about your well-being, and can help you coordinate between your treatment provider and your OB/GYN provider.
